Connect each module's quiz to Microsoft Forms

Each module below can end in a short quiz that's actually a Microsoft Form, embedded right in the page. Because the form lives on forms.office.com, Microsoft — not this page — is what records who completed it.

1 · Build one form per module

  1. In Microsoft Forms, create a new form named something like "Phare Audit L1 — Module 1 Quiz."
  2. Open Settings and set "Who can fill out this form" to "Only people in my organization can respond." This is what unlocks identity capture — an anonymous/public form can't record who answered.
  3. Still in Settings, turn on "Record name." Every submission is now tagged with the responder's org identity automatically — no name field for the rep to fill in.
  4. Add 3–5 short questions. Use the suggested set under each module's quiz (click "Suggested questions" to expand it) as a starting point, or write your own.

2 · Get the embed link

  1. Click Share → the <> Embed tab.
  2. Copy the link — it's the src="…" value inside the iframe code Forms gives you, something like https://forms.office.com/Pages/ResponsePage.aspx?id=….

3 · Paste it in below

Paste each module's link into its field. This is saved in your browser's local page storage, not sent anywhere — so if you're editing this file to hand off to your team, bake the links into the code instead (the FORM_URLS object near the top of the script).

Saved — reopen a module to see its embedded quiz.

Where completion data actually lives

Open any form in Microsoft Forms → Responses tab to see who finished it and when, with their org name attached. Click Open in Excel to export a roster, or wire a Power Automate flow to that form so each new response drops into a shared tracking sheet or Teams channel the moment someone finishes a module.

The progress bar in the sidebar is a courtesy for whoever's reading on this device — it's stored locally, not shared with anyone. Microsoft Forms is your real completion record.
